Support Coordination
Support Coordination is a Division-funded service that assists individuals in gaining access to needed program and State plan services, as well as needed medical, social, educational and other services.
Our experienced and qualified support coordinators
Our experienced and qualified support coordinators will help you get the services you need. We will assign you a trained and dedicated support coordinator who will help you to identify services that you need, draft an Individualized Service Plan (ISP), and ensure that you are receiving the services you need on an ongoing basis. Your assigned individual Support Coordinator will also answer your questions, and respond to emergencies.

To enroll in and receive services through DDD’s Supports Program, an individual must first:
(1) Apply for DDD services and be determined eligible for DDD services
(2) Complete the NJ CAT assessment survey
(3) Maintain Medicaid Eligibility
(4) When notified by DDD, complete the Support Coordination Agency Selection Form
(5) When notified by your Support Coordinator, enroll in the Supports Program by signing the Supports Program Participant Enrollment Agreement
